ABSTRACT

Los errores de refracción contribuyen como un gran problema de salud pública. Las crecientes tasas de prevalencia de la miopía han alcanzado los niveles de epidemia en varias áreas. Este estudio tuvo como objetivo determinar las tasas de prevalencia de errores de refracción en los estudiantes de medicina. Se realizó un estudio prospectivo en el Departamento de Oftalmología, durante un período de un año desde enero de 2020 hasta diciembre de 2020. Los estudiantes de medicina fueron seleccionados al azar, como un total de 250 sujetos. Todos se sometieron a un examen oftálmico. De 250 estudiantes, se documentaron 148 (59.2%) casos de RE. La mayoría de los casos comunes eran mujeres (89, 60.1%), mientras que los hombres eran (59, 39.9%). De estos, 98 estudiantes tienen miopía (66.2%), 42 casos tienen astigmatismo (28.4%) y 8 casos tienen hipermetropía (5.4%). La etapa tardía de las clases (sexto) tiene los casos RE más frecuentes. La miopía fue la RE predominante entre los estudiantes de medicina. La clase anticipada y la mujer son factores predisponentes.


Refractive errors are contributing as a major public health problem. The increasing prevalence rates of myopia have reaching to epidemic levels in several areas. This study was aimed to determine the prevalence rates of refractive errors in medical students. A prospective study was conducted in the Department of Ophthalmology, over a period of one year from Jan 2020 to Dec 2020. Medical students were randomly selected, as a total of 250 subjects. All were undergoing an ophthalmic examination. Of 250 students, 148(59.2%) cases of RE were documented. Most common cases were female (89, 60.1%), whereas male was (59, 39.9%). Of these, 98 students have myopia (66.2%), 42 cases have astigmatism (28.4%) and 8 cases have hypermetropia (5.4%). The late stage of classes (6th) has the most frequent RE cases. Myopia was the predominant RE among the medical students. Advance class and female are predisposing factors

ABSTRACT

Objetivos: describir el comportamiento del defecto miópico hasta el año de vida, en pacientes que desarrollaron retinopatía de la prematuridad, y determinar los principales factores de riesgo del mismo. Método: se realizó un estudio descriptivo, longitudinal, prospectivo en 37 pacientes que, habiendo sido tratados o no por retinopatía de la prematuridad, mantuvieron un seguimiento estable en consulta posterior a la fase aguda de la enfermedad. Fueron evaluadas a los seis y 12 meses de vida teniendo en cuenta las variables edad gestacional al nacer, peso al nacer, tipo de retinopatía de la prematuridad, presencia de miopía. Resultados: la mediana de edad gestacional y peso al nacer fueron 30,2 semanas y 1 287 g. A los seis meses cinco ojos de 27 pacientes (considerando solo los ojos derechos) presentaron miopía, lo cual representa el 18,5 por ciento. A los 12 meses este valor se incrementó a 44,4 por ciento. La edad gestacional y el peso al nacer no mostraron relación con el desarrollo de miopía (p = 0,072 y p = 0,397) a los 12 meses. En pacientes con retinopatía de la prematuridad grave el defecto miópico resultó más frecuente (p = 0,003). Conclusiones: la prematuridad y el peso al nacer no constituyeron variables que influyeran en la aparición de miopía. Por el contrario, la retinopatía de la prematuridad grave sí se asoció con una mayor miopía, sobre todo a los 12 meses(AU)


Objectives: to describe the behavior of the myopic defect up to one year of life, in patients who developed retinopathy of prematurity, and to determine the main risk factors for it. Method: a descriptive, longitudinal, prospective study was carried out in 37 patients who, having been treated for retinopathy of prematurity or not, maintained a stable follow-up in consultation after the acute phase of the disease. They were evaluated at 6 and 12 months of life taking into account the variables gestational age at birth, birth weight, type of retinopathy of prematurity, presence of myopia. Results: Median gestational age and birth weight were 30.2 weeks and 1287g. At 6 months, 5 eyes of 27 patients (considering only the right eyes) presented myopia, which represents 18.5 percent. At 12 months this value increased to 44.4 percent. Gestational age and birth weight were not related to the development of myopia (p = 0.072) and p = 0.397) at 12 months. In patients with severe retinopathy of prematurity, myopic defect was more frequent (p = 0.003). Conclusions: prematurity and birth weight were not variables that influenced the appearance of myopia. On the contrary, severe retinopathy of prematurity was associated with greater myopia, especially at 12 months(AU)

ABSTRACT

Objetivo: Evaluar los resultados refractivos y visuales post cirugía refractiva corneal con láser de excímero (LASEK-MMC o PRK-MMC) en pacientes miopes con o sin astigmatismo asociado. Método: Se realizó un estudio preexperimental del tipo antes y después, en el que fueron incluidos 81 pacientes (162 ojos) tratados con LASEK-MMC (65 pacientes) o PRK-MMC (16 pacientes) seguidos por tres meses. Se les realizó un examen oftalmológico completo y se evaluaron las variables: edad, sexo, diagnóstico y tipo de miopía, así como agudeza visual, esfera, cilindro, equivalente esférico precirugía y poscirugía. Además, se analizó la función visual y los resultados de la refracción. Resultados: El valor de la mediana de edad fue de 24,0 años en los pacientes tratados con LASEK-MMC y en el grupo con PRK-MMC fue de 23,0 años. En ambos grupos existió un predominio del sexo femenino. La totalidad de los pacientes tratados presentaba un astigmatismo miópico compuesto con niveles de miopía leve. A los tres meses el 96,9 por ciento de los pacientes tratados con LASEK-MMC y el 93,8 por ciento de los tratados con PRK-MMC tenían una agudeza visual no corregida de 20/20 o más y todos tenían una visión de 20/40 o más. Conclusiones: La cirugía fotoablativa con láser tiene buenos resultados refractivos y visuales ya que la mayoría de los pacientes quedaron emétropes con una marcada mejoría de la agudeza visual sin corrección y de la refracción en el posoperatorio(AU)


Objective: To evaluate refractive and visual outcomes after excimer laser corneal refractive surgery (laser-assisted subepithelial keratectomy [LASEK] or photorefractive keratectomy [PRK], with intraoperative application of mitomycin-C [MMC]) in myopic patients with or without associated astigmatism. Methods: A preexperimental, before-and-after study was carried out with 81 patients (162 eyes) treated with LASEK-MMC (65 patients) or PRK-MMC (16 patients) and followed for three months. A complete ophthalmologic examination was done and the following variables age, sex, diagnosis and type of myopia were assessed, as well as pre-surgery and post-surgery visual acuity, sphere, cylinder and spherical equivalent. In addition, visual function and the refraction results were analyzed. Results: The median age was 24.0 years in the LASEK-MMC group and 23.0 years in the PRK-MMC group. In both groups there was a predominance of the female sex. All treated patients had compound myopic astigmatism with mild myopia. At three months, 96.9 percent of patients treated with LASEK-MMC and 93.8 percent of patients treated with PRK-MMC had uncorrected visual acuity of 20/20 or better, and all had vision of 20/40 or better. Conclusions: Laser photoablative surgery has good refractive and visual outcomes as most patients became emmetropic with marked improvement in uncorrected visual acuity and refraction postoperatively(AU)

ABSTRACT

Abstract The presence of retinal myelinated nerve fibers is not a rare finding during routine examinations, and it is usually a benign and isolated finding. However, in some rare cases, it can be associated with other ophthalmological conditions. We describe a case of a patient with the triad myelin nerve fibers, myopia and ambliopia, which configures the Straatsma Syndrome.


Resumo A presença de fibras de mielina é um achado comum durante exames oftalmológicos de rotina. Na maior parte das vezes, tem caráter beningno e é um achado isolado. No entanto, em alguns raros casos, a presença de mielinização pode estar associada a outras condições oftalmológicas. Descrevemos um caso de paciente com a tríade presença de fibras nervosas retinianas mielinizadas, miopia, e ambliopia, configurando a síndrome de Straatsma.

RESUMEN Objetivo: Identificar las características de los pacientes pediátricos con miopías por encima de 6 dioptrías. Métodos: Se realizó un estudio descriptivo y observacional, de corte longitudinal, en 43 niños entre 3 y 15 años de edad con diagnóstico de miopía igual o mayor de 6 dioptrías en uno o ambos ojos (72 ojos), quienes acudieron a la consulta del Servicio de Oftalmología Pediátrica y Estrabismo del Instituto Cubano de Oftalmología "Ramón Pando Ferrer" durante el periodo comprendido entre enero y diciembre del año 2018. Las variables del estudio fueron la edad, el sexo, el color de la piel, la miopía (isometrópica y anisometrópica), la longitud axial, la agudeza visual con corrección, el tipo de corrección óptica, la presencia y el grado de estereopsia. Resultados: Predominó el grupo de 3 a 5 años (34,4 por ciento), el sexo femenino y el color de la piel blanca. La longitud axial aumentó junto con el equivalente esférico de los pacientes. La corrección más usada correspondió a los cristales, aunque se demostró estadísticamente que con los lentes de gas permeables los pacientes presentaban mejor agudeza visual (p= 0,016), donde se registró también la media más alta de equivalente esférico. De 43 pacientes, 20 (46,5 por ciento) no presentaron estereopsia. Los pacientes con anisometropías (64 por ciento) no presentaron estereopsia, lo que fue estadísticamente significativo. Conclusiones: Con los lentes de gas permeables los pacientes presentan mejor agudeza visual y mayor equivalente esférico. La mayor cantidad de pacientes sin estereopsia corresponde a las miopías por anisometropías(AU)


ABSTRACT Objective: Identify the characteristics of pediatric patients with greater than 6 diopter myopia. Methods: An observational descriptive longitudinal study was conducted of 43 children aged 3-15 years diagnosed with myopia equal to or greater than 6 diopters in one or both eyes (72 eyes) who attended the Pediatric Ophthalmology and Strabismus Service at Ramón Pando Ferrer Cuban Institute of Ophthalmology from January to December 2018. The variables considered were age, sex, skin color, myopia (isometropic or anisometropic), axial length, corrected visual acuity, type of optical correction, and presence and degree of stereopsis. Results: A predominance was found of the 3-5 years age group (34.4 percent), female sex and white skin color. Axial length increased with spherical equivalent. The most common correction was glasses, but it was statistically demonstrated that gas permeable lenses provided better visual acuity (p= 0.016) and the highest spherical equivalent mean. Of the 43 patients evaluated, 20 (46.5 percent) did not present stereopsis. Patients with anisometropia (64 percent) did not present stereopsis, which constituted a statistically significant fact. Conclusion: Gas permeable lenses provide better visual acuity and a greater spherical equivalent. Most of the patients without stereopsis have anisometropic myopia(AU)

Resumo Objetivo: Identificar a prevalência de alterações visuais em crianças de cinco anos em escolas públicas de Curitiba-PR. Métodos: As escolas foram selecionadas aleatoriamente dentro do município de Curitiba. As crianças com cinco anos completos em 2017 foram avaliadas com a tabela de Snellen, através de distância mínima correta para nitidez de imagem e teste de Hirschberg. Os pais responderam um questionário sobre uso de telas, sintomas oculares e histórico familiar da criança. Os resultados das avaliações foram analisados estatisticamente considerando nível de significância p≤0,05. Resultados: Em uma população de 459 crianças triadas, 219 (47,7%) pertenciam ao sexo feminino e 240 (52,3%) masculino, sendo que do total, 100 foram encaminhadas para avaliação oftalmológica especializada. A partir da triagem observou-se a prevalência de miopia de 10,7%, hipermetropia de 17,6% e estrabismo de 0,9%. Houve relação entre genitores com miopia e filhos míopes (p<0,05). Dentre as queixas oftalmológicas predominaram cefaleia (30,4%) e franzir de testa (10%). Conclusão: A prevalência de alterações visuais encontrada foi de 21,8%. A relação entre distúrbios visuais e o histórico familiar se mostrou estatisticamente significativa. Entretanto, apenas o tempo médio em frente à televisão apresentou influência, dentre os hábitos de vida, sobre as alterações da AV (p=0,028). Queixas oftalmológicas apesar de frequentes, não apresentaram correlação expressiva com a diminuição da acuidade visual.


Abstract Objective: To identify the incidence of visual impairment in 5-year-old children in public schools from Curitiba-PR. Methods: A selection of schools has been choosen randomly from Curitiba. The children, with completed 5 years at end of 2017 have been evaluated using Snellen table, trough minimum distance for image sharpness and Hirschberg test. Parents answered a questionnaire about the use of screens, ocular symptoms and family history of the child. Significance levels were defines as begin p≤0,05. Results: The results have shown that four hundred fifty-nine children were screened. Two hundred nineteen are female (47,7%) and two hundred fourty (52,3%), male. From all screened patients, one hundred were refered to specialized oftalmic evaluation. After trial completing, has been attained a prevalence value of 10,7% for myopia, 17,6% of hyperopia and 0,9% of strabismus. Strong correlation between parents and children has been undiscovered (p<0,05). From listed oftalmic complaints, headache (30,4%) and frown (10%) where most prevalent. Conclusion: It has been attained that the prevalence of visual acuity is 21,8%. The relation between visual acuity alteration and familiar history has been shown to be significant related. On the other side, the average time in front of television has been shown the only habit that has correlation with visual acuity reduction (p=0,028). Vision complaints, although very frequent, doesn't translate into increased probability of visual acuity alteration.

ABSTRACT

Objetivo: determinar la frecuencia de complicaciones en pacientes con miopía axial alta y catarata, a los que se les realizó extracción de catarata por facoemulsificación. Diseño del estudio: observacional descriptivo de frecuencia. Método: se incluyeron 107 ojos de pacientes con miopía axial alta, a los que se les realizó extracción de catarata por facoemulsificación más implante de LIO, operados en 2015. Se realizó una revisión de historias clínicas y descripciones quirúrgicas de los pacientes seleccionados y se evaluó la presentación de complicaciones con un seguimiento a un año después de la cirugía. Resultados: 2 ojos presentaron ruptura de cápsula posterior (1,8 %). Otros 2 ojos presentaron diálisis zonular (1,8 %) y en 3 ojos se evidenció pérdida de vítreo (2,7 %). Durante el primer año postoperatorio de seguimiento se observó desprendimiento de retina en 1 ojo (0,9 %). No se reportaron casos de luxación del LIO a cámara anterior. Conclusión: La frecuencia de complicaciones es similar a la reportada en la literatura mundial e inclusive más baja en algunas variables como la ruptura de capsula posterior y el desprendimiento de retina.


Background: Cataract extraction by phacoemulsifi cation plus intraocular lens (IOL) implant is the most used treatment against the main cause of reversible blindness in the world; the cataract. It is a very successful procedure due to its low frequency of complications and its high reproducibility. However, patients with high axial myopia do not behave in this way and tend to present more complications. Objective: to determine the frequency of complications in patients with high axial myopia and cataract, who underwent cataract extraction by phacoemulsifi cation. Study design: observational descriptive. Method: 107 eyes of patients with high axial myopia were included. All underwent cataract extraction by phacoemulsification plus IOL implantation, in 2015. A review of clinical histories and surgical descriptions of all patients was performed to determine the frequency of complications during a follow-up time of one year aft er surgery. Results: 2 eyes presented posterior capsule rupture (1.8%). Two other eyes presented zonular dialysis (1.8%) and 3 eyes had vitreous loss (2.7%). During the first postoperative year of follow-up, retinal detachment was observed in 1 eye (0.9%). No cases of IOL dislocation to the anterior chamber were reported. Conclusion: The frequency of complications in this population is similar to the one reported in other studies and even lower in some variables such as posterior capsule rupture and retinal detachment.

ABSTRACT

La alta miopía es aquella igual o mayor a -6 dioptrías esféricas, no en función de la curvatura corneal elevada, como es el caso del queratocono, ni por la esclerosis del cristalino que ocurre en etapas preseniles, sino en dependencia de la longitud axial mayor de 25,5 o 26,0 mm. La corrección óptica de estos defectos altos es difícil, ya que es frecuente que los pacientes se sientan incómodos con el uso de gafas, pues no obtienen una buena calidad de visión, y con el fin de mejorarla se han desarrollado distintos procedimientos quirúrgicos. Se presenta una paciente de 26 años de edad con miopía elevada, quien no se adapta a los lentes de contacto y no desea continuar con los espejuelos; no es candidata a la cirugía refractiva corneal. Se le implantaron lentes fáquicas ACR IOL-128 (Soleko, Italia) sin complicaciones. El equivalente esférico preoperatorio -11,5 y -6 dioptrías disminuyó a -1,00 y -0,5 respectivamente, y la agudeza visual sin cristales de cuenta dedos y 0,1 a 0,8 y 0,9 respectivamente. Con este caso se evidencia que las lentes fáquicas son una opción con buenos resultados en la corrección de la alta miopía(AU)


The high myopia, by definition, is that equal or bigger to -6 spherical dioptrías; not in function of the bend high corneal, like it is the case of the queratocono, neither for the sclerosis of the crystalline lens that happens in stages preseniles, but in dependence of the axial longitude bigger than 25,5 or 26,0 mm. The optic correction of these high defects is difficult, since it is frequent that the patients feel uncomfortable with the glasses use, because they don't obtain a good quality of vision and with the purpose of improving it, different surgical procedures have been developed. Young patient 26 years old is presented with high myopia that doesn't adapt to the contact eyeglasseses and she doesn't want to continue with the espejuelos, she is not candidate to surgery refractive corneal and she is implanted eyeglasseses fáquica ACR IOL-128 (Soleko, Italy) without complications. The equivalent spherical preoperatorio -11,5 and -6 dioptrías diminish at -1,00 and - 0,5 respectively and the visual sharpness without glasses of bill fingers and 0,1 at 0,8 and 0,9 respectively. With this case it is evidenced that the lenses fáquicas are an option with good results in the correction of the high myopia(AU)

Resumo Objetivo: Demonstrar a eficácia do uso do colírio de atropina 0,025% em crianças míopes, no Brasil, para a diminuição da progressão da miopia. Métodos: Realizou-se estudo prospectivo em 60 pacientes do Hospital Geral Universitário e Oftalmocenter Santa Rosa - Cuiabá - MT, com idades entre 6 e 12 anos, com equivalente esférico da refração entre -1,00 a -6,00 DE, refração cilíndrica < -1,00 DC e taxa de progressão anual de 0,50 DE (ou maior). Efetuou-se exame oftalmológico geral, topografia corneana e a medida do diâmetro anteroposterior do globo ocular (DAP). Os pacientes foram divididos em dois grupos: em que o Grupo 1 recebeu colírio de atropina 0,025%, todas as noites, e prescreveu-se a refração total com lentes com antirreflexo de multicamadas; e, no Grupo 2, somente a refração total. Nova avaliação foi realizada dois anos após. O teste T Student pareado foi utilizado para comparações das refrações, DAP e ceratometrias, medidas no exame inicial e no exame com 2 anos de seguimento. Resultados: Das 60 crianças, 30 eram do Grupo 1 com idade média de 8,21 ± 1,72 anos, e as do grupo controle com idade média de 8,17 ± 1,73 anos. Quatorze (46,66%) e 16 (53,33%) eram do sexo masculino nos Grupos 1 e 2, respectivamente. O Grupo 1 revelou menor progressão da miopia (Grupo 1: 0,43 ± 0,19D, Grupo 2: 1,24 ± 0,37D) e menor crescimento do DAP em relação ao grupo controle (Grupo 1: 0,19 ± 0,09mm, Grupo 2: 0,48 ± 0,12mm). Houve diferença estatisticamente significativa (P<0,05) entre o grupo tratado e o controle em relação à refração e ao crescimento DAP. A topografia não teve mudança estatisticamente significativa. Conclusão: A atropina em baixas concentrações foi eficaz em diminuir a progressão da miopia em 65% desta população estudada, por 2 anos. No entanto estudos com maior número de participantes e em diversas regiões do Brasil poderiam demonstrar melhor esse fato.


Abstract Purpose: To demonstrate the efficacy of 0.025% atropine eyedrops in myopic children in Brazil for decreasing myopia progression Methods: This was a prospective study with 60 children from Hospital Geral Universitário and Oftalmocenter Santa Rosa in Cuiabá, MT, Brazil, aged between 6 to 12 years, with spherical equivalent refractive error of -1.00 to -6.00 diopters (D) and astigmatism of -1.00 D or smaller. They underwent a complete ophthalmological examination, corneal topography and optical biometry. Children were assigned into two groups: group 1 used 0.025% atropine drop, once-nightly dosing, and it was prescribed total refraction in anti-reflective coating lens; and group 2 was prescribed just total refraction. A new evaluation was conducted 2 years after that. Paired student's t-test was used to compare refractions, axial length and keratometry which were measured in an initial exam and after a two-year follow-up. Results: Of the 60 children, the 30 in group 1 had an age mean and SD 8.21 +/- 1.72, and of the control group were 8.17 +/- 1.73 years. Fourteen (46,66%) and 16 (53,33%) were male, respectively. Myopic progression was significantly lower in group 1 (-0.43 +/- 0.19 D) than in group 2 (-1.24 +/- 0.37 D) and axial length increase was also significantly smaller in group 1(0.19 +/- 0.09 mm) than in group 2 (0.48 +/- 0.12 mm). There were no significant statistical differences regarding keratometry between groups. Conclusions: Low dose atropine eyedrops were effective in decreasing myopia progression in 65% of this population studied for 2 years. Furthermore, a larger scale randomized controlled study with longer follow-up seems warranted.

ABSTRACT

Abstract This article reports two cases of Straatsma Syndrome, a rare disease, emphasizing its clinical features that inclued myopia, strabismus and amblyopia associated with persistent myelinated fibers in the retina. Ophthalmic examination, color retinography and optical coherence tomography were performed.


Resumo Este artigo relata dois casos de síndrome de Straatsma, uma doença rara, enfatizando suas características clínicas que incluem miopia, estrabismo e ambliopia associada a persistência de fibras de mielina na retina. Foram realizados exame oftalmológico, retinografia e tomografia de coerência óptica.

ABSTRACT

La emergencia de las lentes intraoculares fáquicas en las pasadas décadas mostró una nueva alternativa para las ametropías extremas. Desde el año 1950 han sido diseñados diferentes modelos de lentes intraoculares fáquicas con distintos diseños y complicaciones. Se presenta un caso con antecedente de miopía elevada, quien posterior a la colocación de una lente intraocular fáquica de cámara anterior de fijación angular presentó como complicación en el posoperatorio abundante pigmentación iridiana sobre el lente y sinequias anteriores en 360 grados. Nos motivamos a publicar este caso y de esta forma proporcionar una actualización sobre las complicaciones de la colocación de estos lentes, así como ofrecer algunas recomendaciones para detectar a tiempo los posibles riesgos de este procedimiento(AU)


The emergence of the phakic intraocular lenses in the last decades showed a new alternative for the extreme ametropias. Since1950 different models of phakic intraoduclar lenses have been designed with different designs and complications. Here is a patient with a history of high myopia who, after having undergone the implantation of an anterior chamber phakic intraocular lens with angle fixation, presented with abundant iridian pigmentation over the lens and anterior sinechia at 360 degrees. The objectives of this report were to provide an update on the complications of the implantation of these lenses as well as making some recommendations to timely detect the possible risks of this procedure(AU)

RESUMO Objetivo: Avaliar o aumento da prevalência de miopia entre pacientes atendidos em períodos diferentes em um serviço oftalmológico de Goiânia. Métodos: Foram comparados dados coletados em dois estudos científicos realizados em um serviço de oftalmologia, em diferentes períodos de tempo, que avaliaram dentre outros fatores, os erros refracionais dos participantes a partir destes dados comparou-se a porcentagem de pacientes portadores de miopia presentes nos estudos. Resultados: Observou-se um aumento na prevalência de Miopia dentre os participantes dos dois estudos. Na avaliação realizada entre 1995 e 2000 a porcentagem de pessoas com miopia representou 3,6% do total, já no estudo realizado no ano de 2014 a prevalência de pacientes com miopia foi de 9%. Dentre os pacientes com erros refracionais a prevalência de miopia no primeiro estudo foi de 9,97%, já no segundo a prevalência foi de 22%. Conclusão: Observou-se um aumento na prevalência de miopia entre os dois estudos, dados esses que corroboram com análises feitas que mostram um aumento da prevalência de miopia em todo mundo nos últimos 30 anos. Porém em uma proporção menor do que a observada em outros estudos populacionais.


ABSTRACT Objective: To evaluate the increased prevalence of myopia among patients in different periods in an ophthalmologic hospital in Goiânia. Methods: There were compared data collected in two scientific studies carried out in the same ophthalmologic hospital in different periods of time, they evaluated among other factors, the refractive errors of the participants, from this data there were compared the percentage of patients with myopia between the two studies. Results: There was an increase in the prevalence of myopia among the participants of the studies, in the evaluation carried out between 1995 and 2000 the percentage of people with myopia accounted for 3.6% of the total, in the study carried out in 2014 the prevalence of patients with myopia was 9%. Only among patients with refractive errors the prevalence of myopia in the first study was 9.97%, in the second study the prevalence was 22%. Conclusion: There was an increase in the prevalence of myopia between the two studies, these data corroborate with tests that shows an increasing prevalence of myopia in the world in the last 30 years. However, in a smaller proportion than in other population studies.

ABSTRACT

The authors report a 22-year patient presented with night-blindness and progressive visual loss. Ophthalmologic examination included slit lamp biomicroscopy, best-corrected distance visual acuity, cycloplegic refraction, A-mode and B-mode ultrasonography, and full-field flash electroretinogram [ERG]. Cycloplegic refractions were -6.00 [-1.25 x 175] dpt in the right eye and -7.00 [-1.50 x 165] dpt in the left eye. Slit lamp examination showed normal anterior segment. Fundoscopic examination revealed bone-spicule pigmentations, waxy optic disc, arteriolar narrowing and posterior staphyloma, with 5 optic disc in diameter in the right eye and 4 optic disc diameter in the left. Ocular ultrasonography confirmed staphyloma. Full-field ERG showed evidence of a generalized retinal dysfunction involving both rod and cone responses, supported the diagnosis of retinitis pigmentosa

ABSTRACT Purpose: To evaluate the difference in terms of refractive errors and anterior segment parameters between schizophrenic patients and healthy volunteers. Methods: This study compared 70 patients (48 men) who were diagnosed with schizophrenia with a control group of 60 (35 men) who were similar in terms of age, gender, education, and socioeconomic level. Anterior segment examination was performed using a Scheimflug system. Axial length and lens thickness (LT) were measured using optic biometry. The following tests were administered to the psychiatric patient group: Brief Psychiatric Rating Scale (BPRS), Scale for the Assessment of Negative Symptoms (SANS), and Scale for the Assessment of Positive Symptoms (SAPS). Results: Mild myopia was detected in both the schizophrenic and control groups, with no statistically significant difference (p>0.005). Corneal volume (CV), anterior chamber volume (ACV), anterior chamber depth (ACD), and central corneal thickness (CCT) values were lower in the schizophrenic group, and there was a statistically significant between-group difference (p=0.026, p=0.014, p=0.048, and p=0.005, respectively). LT was greater in schizophrenics, and the difference was found to be statistically significant (p=0.006). A statistically significant negative correlation was found between SAPS and cylinder values (p=0.008). The axial eye length, cylinder value, pupil diameter, mean keratometric value, and anterior chamber angle revealed no statistically significant difference between the groups (p>0.05). Conclusion: No statistically significant difference was detected in terms of refraction disorders between schizophrenics and the healthy control group, while some differences in anterior chamber parameters were present. These results demonstrate that schizophrenics may exhibit clinical and structural differences in the eye. .


RESUMO Objetivo: Avaliar se existem diferenças em relação aos erros refracionais e parâmetros do segmento anterior entre pacientes com esquizofrenia e voluntários saudáveis. Métodos: Este estudo comparou 70 pacientes diagnosticados com esquizofrenia (48 homens) com um grupo controle de 60 pacientes, semelhantes em relação à idade, sexo, escolaridade e nível socioeconômico (35 homens). O exame do segmento anterior foi realizado com o sistema Scheimflug; os comprimentos axiais do olho e a espessura do cristalino foram avaliadas por meio de biometria óptica. Os seguintes testes foram aplicados ao grupo de pacientes psiquiátricos: Brief Psychiatric Rating Scale (BPRS), Scale for the Assessment of Negative Symptoms (SANS), e Scale for the Assessment of Positive Symptoms (SAPS). Resultados: Miopia leve foi detectada em ambos os grupos de esquizofrenia e de controle, sem diferença estatisticamente significativa (p>0,005). Volume de córnea (CV), volume da câmara anterior (ACV), profundidade da câmara anterior (ACD) e paquimetria central da córnea (CCT) apresentaram valores menores no grupo de esquizofrênicos e houve diferença estatisticamente significativa entre os dois grupos (p=0,026, p=0,014, p=0,048 e p=0,005, respectivamente). A espessura do cristalino (LT) foi maior em esquizofrênicos e a diferença foi estatisticamente significativa (p=0,006). Foi encontrada uma correlação negativa estatisticamente significativa entre SAPS e os valores cilíndricos (p=0,008). O comprimento axial do olho, o valor do cilindro, o diâmetro pupilar, a ceratometria média e o ângulo da câmara anterior não revelaram nenhuma diferença estatística entre os grupos (p>0,05). ABSTRACT

We report a case of a 66-year-old man with a history of high myopia and who was referred for acute decreased visual acuity of the right eye. Fundus examination and optical coherence tomography (OCT) showed a mild epiretinal membrane (ERM) and splitting of retinal layers. Pars plana vitrectomy was performed with intravitreous triamcinolone injection, posterior hyaloid and ERM peeling, and 12% perfluoropropane (C3F8) gas tamponade. After remaining asymptomatic for 17 months, the patient reported a new episode of sudden decreased visual acuity in his right eye, and OCT showed recurrent myopic foveoschisis (MF). He underwent vitrectomy and internal limiting membrane (ILM) peeling. Six months later, the patient’s best corrected visual acuity had improved to 20/25. Optical coherence tomography showed a remarkably improved macular anatomy, with residual traction along the inferotemporal arcade, which was attributed to the vessel stiffness itself. We conclude that removing the internal limiting membrane is a challenging maneuver in myopic foveoschisis, even with staining approaches. Although myopic foveoschisis may be resolved without peeling the internal limiting membrane, its removal should be considered if the condition recurs.


Relatamos caso de um homem de 66 anos, com antecedente de alta miopia, que referiu baixa de acuidade visual aguda no olho direito. O mapeamento de retina e o exame de tomografia de coerência óptica (OCT) mostraram discreta membrana epirretiniana (ERM) e isquise retiniana. Foi realizada vitrectomia via pars plana com injeção intravítrea de triancinolona, retirada da hialóide posterior, peeling da membrana epirretiniana e tamponamento com gás perfluorpropano (C3F8) a 12%. O paciente permaneceu assintomático por 17 meses, quando queixou-se de novo episódio de baixa de acuidade visual súbita no olho direito e o tomografia de coerência óptica mostrou recorrência da isquise miópica. Ele foi submetido a nova vitrectomia com peeling da membrana limitante interna (ILM). Após 6 meses, a acuidade visual corrigida era de 20/25. A tomografia de coerência óptica mostrou melhora importante da anatomia macular, com área de tração residual observada na arcada inferotemporal, que foi atribuída à rigidez do próprio vaso. A retirada da membrana limitante interna é uma manobra desafiadora em olhos alto míopes, mesmo estando corada. A resolução da isquise miópica pode ser atingida sem o peeling da membrana limitante interna, mas sua remoção deve ser considerada em casos de recorrência.

ABSTRACT

La evaluación de los discos ópticos en pacientes miopes suele ser problemática, razón por la cual muchas veces el apoyo en imágenes diagnósticas suele ser de gran ayuda. Se describirán algunos casos clínicos en los cuales la evaluación clínica no fue conclusiva, razón por la cual el apoyo en la tomografía óptica coherente fue determinante. El equipo RS-3000 Nidek ® usa una base normativa, realizada en pacientes miopes con longitudes axiales superiores a 26mm, los resultados suelen ser distintos especialmente en la evaluación del complejo celular ganglionar. Se describen dos casos clínicos en los cuales la evaluación mediante OCT fue de gran importancia.


The evaluation of optical discs in myopic patients is often problematic, this is why diagnostic imaging support is often helpful. In some clinical cases in which the clinical evaluation was inconclusive, the optical coherence tomography, was decisive. The RS-3000 Nidek ® equipment uses a normative database held in myopic patients with axial lengths between 26 to 29 mm, the OCT results are often different especially in the evaluation of ganglion cell complex. We describe two clinical cases in which the evaluation by OCT was of great importance.

ABSTRACT

PURPOSE: To evaluate choroidal blood flow changes in eyes with high myopia according to the pulsatile components of ocular blood flow analysis. METHODS: A total of 104 subjects (52 males and 52 females) were included in this study. One eye of each participant was randomly selected and assigned to one of four refractive groups, designated as, hyperopes (n = 20; refractive error, > or =+1.00 diopter [D]), emmetropes (n = 28; refractive error, +/-0.75 D), lower myopes (n = 33; refractive error, -1.00 to -4.75 D), and high myopes (n = 23; refractive error, < or =-5.00 D). Components of pulse amplitude (OBFa), pulse volume (OBFv), pulse rate (OBFr), and pulsatile ocular blood flow (POBF) were analyzed using a blood flow analyzer. Intraocular pressure and axial length were measured. RESULTS: Pulsatile components of OBFa, OBFv, and POBF showed positive correlations with refractive error and showed negative correlations with axial length (r = 0.729, r = 0.772, r = 0.781, respectively, all p < 0.001; r = -0.727, r = -0.762, r = -0.771, respectively, all p < 0.001). The correlations of refractive error and axial length with OBFr were irrelevant (r = -0.157, p = 0.113; r = 0.123, p = 0.213). High myopes showed significantly lower OBFa, OBFv, and POBF than the other groups (all p < 0.001). CONCLUSIONS: Axial length changes in high myopes potentially influence choroidal blood flow, assuming the changes are caused by narrowing of the choroidal vessel diameter and increasing rigidity of the choroidal vessel wall. These finding explains the influence of axial length on OBFa, OBFv, and POBF, but not on OBFr. Thus, changes in axial length and the possible influence of these changes on the physical properties of choroidal vessels is the mechanism believed to be responsible for putting high myopes at risk for ocular vascular diseases.

ABSTRACT

PURPOSE: To investigate the biometric risk factors for corneal surface complications associated with hydrogel soft contact lens (SCL) fitting in myopic patients in Korea. METHODS: This is a retrospective case-control study. The records of 124 subjects (124 eyes) who wore SCLs on a daily basis were reviewed. Thirty-one patients (31 eyes) who were diagnosed with corneal neovascularization (NV) while wearing SCLs were included in the complication group. Ninety-three age- and sex-matched patients (93 eyes) who wore SCLs, who did not have corneal NV and who visited our clinic for correction of refractive errors were included in the control group. The degree of spherical equivalent, astigmatism and corneal base curve radius (BCR) were compared in both groups. RESULTS: Patients with NV exhibited poorer best corrected visual acuity and more myopia than controls (p = 0.008 and 0.006, respectively). In univariate analysis, highly myopic patients (-9 diopters [D] or higher) were more likely to experience NV (odds ratio [OR], 2.232; 95% confidence interval [CI], 1.602 to 3.105). High astigmatism (> or =2 D) increased the risk of complications (OR, 2.717; 95% CI, 1.141 to 6.451). Steep cornea, in which BCR was <7.5 mm, also raised the risk of complications (OR, 4.000; 95% CI, 1.661 to 9.804). Flat cornea was not a risk factor for the development of NV. CONCLUSIONS: High myopia, high astigmatism, and steep cornea seemed to be risk factors in the development of corneal NV in SCL wearers.

ABSTRACT

PURPOSE: To evaluate the change of accommodation-convergence parameters after implantation of Artisan phakic intraocular lens (PIOL). METHODS: Prospective study for the patients with the Artisan PIOL implantation was performed. A total of 37 patients (3 males and 34 females) enrolled the study. Preoperatively, convergence amplitude, the stimulus accommodative convergence per unit of accommodation (AC/A) ratio and the near point of convergence (NPC) were evaluated. After the Artisan PIOL implantation, the identical evaluations were repeated at 1 week, 1, 3, and 6 months after the surgery. RESULTS: Mean age was 24.3 +/- 4.8 years old, and preoperative refractive error was -8.92 +/- 4.13 diopters (D). After the implantation, mean refractive errors significantly decreased to within +/-1.00 D, and noticeable complications were not found. The convergence amplitude and the stimulus AC/A ratio increased 1 month after the surgery, but progressively stabilized afterward to near preoperative values. NPC didn't show any significant change over follow-up period up to 6 months. CONCLUSIONS: These results regarding implantation of the Artisan PIOL revealed the increase of accommodation-convergence relationship within first 1 month after the surgery, but progressive stabilization was noted during follow-up periods.
